Every time an issue moves without a recorded changelog entry, it is a bypass — a transition that happened outside the defined process. The workflow adherence score measures what proportion of all transitions are properly documented, as a positive governance signal rather than a risk metric.

A high adherence score means the team is following its workflow. A low score means bypass behavior is common — and potentially systematic.

What you can conclude

  • A score above 90 indicates strong workflow discipline — the vast majority of transitions are documented and traceable.
  • A declining score over time indicates that bypass behavior is becoming more common — a process conversation is needed.
  • Comparing adherence scores across projects reveals whether process discipline is consistent across the portfolio or concentrated in specific teams.
